Ultra Membership

$199 per month per member

Includes unlimited visits in your home per month, unlimited telehealth visits, texting directly with provider, same or next day urgent visits as appropriate, annual preventive visit/physical exam scheduled in advance. All required forms and letters included. 

Minimum 1 year membership required. 

One time enrollment fee of $125 waived.

Premium Membership

$149 per month per member

Includes up to 1 visit in your home per month, unlimited telehealth visits, texting directly with provider, same or next day urgent visits as appropriate, annual preventive visit/physical exam scheduled in advance. All required forms and letters included. Additional in person visits will incur additional cost at urgent visit rates. 

Minimum 3 month membership required. 

All plans incur a one time enrollment fee of $125, waived for 1 year committment up front. 

Standard Membership

$99 per month per member

Includes up to 1 visit in your home or by telehealth visit per month including the annual preventive visit, texting directly with provider, annual preventive visit/physical exam scheduled in advance. Phone calls requiring 15 minutes or more of the provider's time will be charged as a virtual/telehealth visit. All required forms and letters included. Additional visits will incur additional cost at urgent visit rates. May convert to Premium membership at any time.

Basic Membership

$49 per month per member

Includes annual preventive/physical exam visit scheduled in advance and up to 2 urgent/sick visits per year either in person or telehealth. Additional visits will incur additional  cost at urgent visit rates. May convert to Standard or Premium membership at any time. Forms and letters subject to $25 administrative fee under provider's discretion, paid in advance.

Additional Visit

Urgent, sick,or extra visits for members and at the providers discretion, we may accept a one time urgent visit for your family member, cost $150. This will include diagnosis treatment and follow up for ailments such as seasonal allergies, insect bites, asthma attacks, ear infection, headache, nausea or vomiting, pink eye, rash or skin irritation, sore throat. Please note, urgent visit is not an option for management of chronic diseases, multiple complaint visits, lacerations, broken bones, chest pain. Forms and letters subject to $25 administrative fee under provider's discretion, paid in advance. 

May convert to any membership and waive enrollment fee. Payment expected at time of visit.

Discounts

• All plans may upgrade at any time, no additional enrollment fee applies. 
• Enrollment fee waived if one year commitment is signed at time of enrollment only, Standard, Premium, and Ultra plans only. 

Payments

One (1) week grace period for payment, after which late payments will incur a $25 fee. We offer automatic payments to avoid this.

Re-enrollment at discretion of the provider, with $50 re-enrollment fee. 

DOT Exam Only

Department of Transportation (DOT) Physical Exams to be offered in your home, scheduled in advance only for members, non members at providers discretion. Insurance not applicable, cost $130 per exam. Exam includes visual acuity and urinalysis as required by FMCSA. Payment expected at time of visit. 

Insurance

Effective October 31, 2025, we will no longer be accepting Aetna health insurance plans. If you have Aetna and are a current patient, we are happy to convert you to a monthly enrollment of your choice, please contact the office to discuss options.

Please note, if your insurance is not accepted and you wish to enroll, you may enroll under one of the subscription plans listed above (MassHealth patients excluded).

Preventive Healthcare

Preventive health screenings are designed for early detection and treatment of certain cancers, diabetes, and heart disease. Education including healthy lifestyle for prevention and management of disease. Annual physical exams are included in all of our membership levels.

Prescription management

There are "five rights" of prescription management including the right patient, drug, time, dose, and route. We not only will make sure that you are on the right medication, but also specialize in de-prescribing techniques which is removing unneccessary medications that may lead to increased interactions and side effects.

Diagnostics

Diagnosis and Treament of common Illnesses including but not limited to diabetes; cholesterol; asthma; COPD;  thyroid disorders; high blood pressure (hypertension); osteoarthritis; anxiety and depression; acute respiratory, ear, eye, urinary and skin infections, back pain, migraines/headaches, sleep apnea, allergies.

Women's Health

Breast and cervical cancer screening counseling and management of menstrual and menopause concerns. Birth control prescription and management. We also can offer referral to gynecology if you prefer. Please note, that while preventive screening pap smear is included with your package, your insurance may bill you for lab costs associated with the test.

Immunizations

Though we do not currently offer immunizations, we are happy to recommend immunizations based on current medical guidelines. We will even send a prescription for the recommended immunizations to your local pharmacy.

Lifestyle Counseling

We wll provide guidance on lifestyle changes such as exercise, diet, and sleep hygiene to promote overall health and prevent the progression of disease, allowing you to be in control of your own health.

Telehealth

We are offering visits both in person in your home as well as virtually through video visit, which can treat a variety of concerns. With your monthly Ultra, Premium, or Standard subscriptions you can also reach your provider by text message to coordinate your care.

Chronic Disease Management

We will manage your chronic diseases through personalized treatment plans geared towards improvement in your overall quality of life. This includes maintenance and prevention of exacerbations of the disease to allow you the highest level of participation in your every day life.

Communication

We offer communication through a variety of means including telephone call directly with your provider, email, text, telehealth, and in person. With a personalized touch, you will feel involved and connected with your care plan and overall well-being.
